1) Understand there isn't time to rectify for this round and appreciate that it will be taken into consideration for next election.
However, I like to point out that the form is printed back to back, thus not quite possible to maintain confidentiality of vote if one can just flip over to see the member's name.
I just hope that doesn't deter anyone from voting.

3) appreciate that anyone handling the ballot forms be reminded of the need for information confidentially.
